- Go to Menu>Settings>Advanced (123, 456, etc.)>Administration Settings>Reset to Defaults>1,2, and 3
- Cancel startup and hold down 1+3+5 and then enter MAC address
- If that doesn't work reboot and restart the phone
- Go to https://ipaddress

- Settings>Provisioning Server
- Server Type: HTTPS
- Server User: Leave Blank
- Server Password: Leave Blank
- Boot Server: Select Custom from the menu
- Boot Server Option: 128
- Click Save
